Overland Park-based WellSky has partnered with a Florida-based Millennium Physician Group and will provide its doctors with technology to improve patient outcomes through post-acute care.
Millennium physicians can access WellSky’s technology, clinical services team, and extensive acute, post-acute, and home care footprint through this partnership allowing for a better value-based care initiative. Millennium has more than 800 healthcare providers across the Southeast.
WellSky’s clinical services team will prioritize longitudinal care coordination with its new partner, allowing Millennium physicians to provide continuous support to their patients in post-acute care facilities and the home, according to a release.
“At WellSky, we recognize the vital role physicians play in value-based care,” Dr. Lissy Hu, president of connected networks at WellSky said in the release. “With this revolutionary collaboration, we’re connecting physicians to acute and post-acute settings of care to improve health outcomes for patients.”
WellSky serves more than 20,000 client sites specializing in innovative healthcare technology which provides post-acute facilities, government agencies, human services organizations and some of the largest US hospital systems. The company employs more than 2,500 full-time workers.
In October, WellSky acquired Corridor, an end-to-end tech-enabled services platform, to bolster its post-acute care services.
