Kansas City-based medication management firm Tria Health has been sold to Stellus Rx, headquartered in Plano, Texas.

Stellus Rx and Tria Health will create a single, pharmacist-led platform designed to support patients’ pharmacy care management, according to a release. Tria Health will continue operating under the same brand in the near term, with no changes to care teams, services or client relationships.

Founded in 2009, Tria Health specializes in helping patients manage their chronic conditions. The firm personalizes care for members, improves health literacy, closes care gaps, and simplifies benefits for employers to improve outcomes and reduce healthcare costs.

“Tria Health and Stellus Rx share a deep commitment to patient-centered, pharmacist-led support,” CEO of Tria Health, Jessica Lea, said in the release. “This marks an exciting next chapter for our team and our clients, allowing us to expand our impact while advancing the care model that has made Tria Health successful.”

Stellus Rx is a pharmacist-led patient support and dispensing solutions company that works with patients to optimize medication plans. Stellus Rx is a portfolio company of WindRose Health Investors, a New York-based investment company with approximately $7 billion under management in the healthcare service industry.

“By bringing Stellus Rx and Tria Health together, we’re creating a pharmacist-led platform that helps organizations realize the full clinical and financial impact of effective medication use, across all types of organizations that care about lowering costs through improved health,” CEO of Stellus Rx, Tony Willoughby, said in the release.
